Case 245

assigned

Report English

Received articulation in the right knee joint. Patella centered while standing. From the previous examination, a known large full-thickness cartilage defect retropatellar on the medial and lateral facet and ridge measuring up to 18 x 15 mm (4/15 and 6/21). Adjacent cortical thinning and slight bone edema as well as small cystic changes (3/42). Individual small osteophytic projections retropatellar (e.g., 6/23). Also in the trochlea, a constant cartilage defect from the previous examination, predominantly on the lateral side, with adjacent subchondral edema and cysts regressing compared to the previous examination (7/21 and 5/12). Constant osteophytic projections of the lateral trochlea from the previous examination (3/20) with adjacent T2 signal elevations subchondrally (5/8). Femorotibial cartilage coverage preserved. Medial meniscus posterior horn slightly T2 signal altered, differential diagnosis of initial degeneration. No meniscal tear formation. Anterior and posterior cruciate ligaments intact. Collateral ligament apparatus also intact. No joint effusion. Accentuated fluid in Baker's position. Increased fluid prepatellar (3/96). Surrounding musculature and subcutis unremarkable. From the external previous examination of 04/24, a known and unchanged full-thickness cartilage defect retropatellar on the lateral and medial facet as well as on the ridge measuring up to 18 x 15 mm (4/15 and 6/21) with adjacent subchondral reactive edema and small cystic changes regressing over time, as well as discrete osteophytes and cortical thinning due to degenerative changes. Also trochlear, predominantly lateral, a constant full-thickness cartilage defect from the previous examination with adjacent subchondral edema and cysts. Discrete degenerative changes also on the lateral femoral condyle. No joint effusion. No free joint body. Accentuated fluid in Baker's position. Increased fluid prepatellar (3/96).
