Anterior shift of the tibia. Old drill channel in the distal femur at the level of the metaphysis. Significant bone marrow edema of the lateral femoral condyle, less so in the medial femoral condyle and the tibial plateau, with the point of maximum intensity dorsolateral. No significant edema signal in the proximal fibula. Centered position of the patella. No significant retropatellar chondropathy, intact depiction of the retinaculae. Significant joint effusion, primarily in the suprapatellar recess. Intact depiction and signal of the posterior cruciate ligament. Intact depiction of the femoral insertion of the ACL graft, but the fibers along the course to the tibial insertion are discontinuous and swollen due to rupture of the graft centrally transligamentous. Slight nodular soft tissue proliferation ventrally adjacent to the tibial insertion of the graft (7/21), differential diagnosis nodular arthrofibrosis. Approximately 10 mm measuring oval free joint body retropatellar (4/15). Unremarkable cartilage covering of the medial femoral condyle. Impression on the lateral femoral condyle consistent with femoral notch (6/27). In this location, focal full-thickness cartilage defect (6/27). Complex tear formation in the posterior horn of the medial meniscus (6/8). Lateral meniscus intact. Collateral ligaments intact. Central transligamentous rupture of the ACL graft. Consecutive anterior tibial shift. Lateral femoral notch with focal full-thickness chondropathy (in the sense of osteochondral lesion). Significant bone bruise of the femoral condyles, primarily lateral, and of the tibial plateau, primarily dorsolateral. PCL and collateral ligaments intact. Complex tear formation in the posterior horn of the medial meniscus (6/8), lateral meniscus intact. Slight arthrofibrosis ventrally adjacent to the tibial insertion of the graft, no larger nodular arthrofibrosis. Free joint body retropatellar measuring 1 cm. Significant effusion suprapatellar.
